| CFHCN’s History |
| In 1975, young medical and behavioral healthcare professionals attending Chicago Universities,
and Circle church, were challenged to use their skills to serve in communities of need.

| These professionals were moved to action by their religious convictions and a concern over the
lack of response to the racial and demographic changes in the Austin neighborhood that led to growing emotional health disparities
and a lack of adequate health care for residents. These professionals were challenged to stay in the Austin community. All moved
into the Austin community and established the first offices of services there, providing counseling and medical. |
| In 1977, Circle Family Care became incorporated as two independent agencies, one addressing the
medical needs and the other addressing the emotional needs. In 1984, these two organizations merged into one and became a 501 (c)
(3) not-for-profit organization under the name Circle Family Care (CFC), with Mr. Lenard Sharber serving as its CEO until he passed
away in 2005. In 1986, Circle Family Care expanded its counseling services to the Humboldt Park community through a contract awarded by The Illinois Department of Mental Health. This expansion allows CFC to service many Hispanic families migrating from their homeland into America’s neighborhoods.

| CFC coordinates with West Suburban, Bethany, Oak Park, and University of Illinois hospitals to
expand services and meet the needs of the community. Believing in whole-person care, the center offers comprehensive medical services
(everything from routine screenings to 24-hour urgent care) as well as counseling and outreach programs including after-school
programs for teens and 24-hour support for mentally ill clients.

| Our Behavioral Health Services Units assists mentally ill adults and provides family support
services in the areas of domestic violence and youth outreach. The Children & Family Services Unit is geared toward child welfare
and providing Christian based counseling, in addition to emotional and structural support to families in need. CFC's Community Health
Services Unit focuses on the physical well being of patients young and old by providing comprehensive, well-rounded health care and
supportive services in a community setting. |
