Mobile Health Unit
Healthcare for the Homeless: A multidisciplinary team for preventive and primary health care to homeless people, which reaches eighteen transitional shelters and drug rehabilitation shelters. Team members assess each patient’s medical and social needs. The Mobile Health Team includes: a family physician, two family nurse practitioners, an outreach worker, mental health social worker and social service advocate.

  • Referrals to Circle Family HealthCare's clinic for regular medical care after leaving the shelter.
HIV/AIDS
We offer an HIV Early Intervention Program designed for the unique needs of those living with HIV/AIDS. HIV educational seminars are provided at churches and community centers. Programs are also given four times per year for clients by the mobile health team. Through collaboration with Heartland Health Outreach, our Mobile Health Homeless Team services thirteen shelters.
Over a decade, epidemiological HIV/AIDS data has shown that Chicago’s Westside maintains some of the highest prevalence rates in Chicago (Silva, 1999). The African American Community suffers great disparities with a higher incidence of HIV and AIDS than other groups. As part of its strategic initiative, CFHCN is pursuing an innovative HIV/AIDS program, provide greater community access for testing and case management. We seek to increase awareness and resources to better address the HIV/AIDS dilemma on the Westside.

Pediatrics
Pediatric care deals with the development, care, and diseases of babies and children. Pediatricians devote much of their time to health examinations, preventive medicine and immunizations for children against infectious diseases like influenza, meningitis, measles, mumps, and chicken pox.
 
Domestic Violence
Is unfortunately another concern for children, which has become a prevalent problem within our society. It is crucial that we provide safety and security for all family members. We need to prevent child abuse and violence against women from reoccurring.
 
Obstetrics and Gynecology
To ensure a healthy life for newborns, we’ve sought to enhance our OB/GYN services. We have a family practitioner/OB provider. CFHCN also provides linkages for Women Infants Children Program (WIC) and family case management services that will increase the birth rate numbers. Our clients regard this service as crucial.
 
Maternal Child Health Care / Prenatal and Perinatal Healthcare
We’re establishing a Women’s and Children’s Health Program. This includes contracting with University of Illinois/Chicago (UIC) for their midwifery program. While these services are well received by patients, the level of need is greater than we presently provide. The Maternal Child Health Program will better meet the needs of the community. We understand that enhancing this service is crucial to its survival.
 
Oral Health / Dental Care Program
CFHCN provides dental screening to all patients. However, that is not enough. As part of our strategic initiative, we’re establishing a dental pilot program, with oral health and dental services at 5120 W. Jackson Boulevard in Chicago.
 
Head Start
CFHCN has partnered with the City of Chicago, Division of Youth Services to provide various health education and medical services to the children and their families, and general health support services to 20 Head Start centers around the city in an effort to increase compliance with school physicals and immunizations. Last year, approximately 900 preschool children and their families were served through Head Start.

  • Workshops and trainings: Lead, immunizations, hygiene, STD’s, OSHA standards, and CPR trainings. Offered to students, parents, and school employees. Services provided by a CFHCN nurse to increase compliance with school physicals and immunizations.
Outreach - Prevention/Education
Our outreach efforts continues to expand! We enroll community residents in Medicaid and Kidcare at the health center and during other community-based activities. Outreach staff has participated in dozens of community health fairs and events. This includes Diabetic, Health Disparities, and Expungement Empowerment Summits. We’ve visited approximately 50 neighborhood churches, 20 schools, and 20 other Community Based Organizations. Interactive education is provided on topics of asthma, HIV/STD prevention, Obesity Prevention, Lead Poisoning Prevention, Smoking Cessation, Substance Abuse, Violence Prevention, and other topics requested by community agencies.
 
Cancer Screening
Prostrate, Cervical, Colon and Breast Cancer Screenings are an essential component of CFHCN’s services, and critical to overall health. These tests are vital in detecting problems and promotes preventive care. A yearly mammogram for women after the age of 35, prostrate check for men, and colon check up for each person, age 50+, will help clients maintain a healthy mindset of holistic care.
